CBFS Rehabilitation Coordinator

JOB DUTIES:

Work as a member of the Collaborative Action for Recovery (CAR) Team to provide community-based rehabilitation, support and supervision services. Provide consultation to clients and other team members on completing functional and strengths-based assessments and implementing Individualized Action Plans (IAP) using rehabilitation values, principles and techniques, including skills training. Provide training to team members to maintain adherence to standards of Medicaid Rehabilitation Option service delivery and documentation. Assist the CBFS Team Leader in maintaining tracking systems to support adherence to state and federal documentation standards for clients served by the team.

SCHEDULE: Monday, Wednesday, Friday 8a-4p, and Tuesday and Thursday 12p-8p.

REQUIREMENTS:

All applicants must be at least 21 years of age or older and hold a valid US drivers license for at least 1 year. Must have access to an operational and insured vehicle and be willing to use it to transport clients. Working as part of a Collaborative Action for Recovery (CAR) team, the Clinical Coordinator will provide clinical expertise and outreach services to adults receiving services under the Department of Mental Health's Community Based Flexible Supports (CBFS) model. The Clinical Coordinator will provide consultation and training to both clients and other team members on mental illness, evidence-based psychiatric treatment and risk reduction strategies. The Clinical Coordinator will complete Assessments, develop Individual Action Plans (IAP) and provide coordination and liaison services for CBFS clients with community and hospital resources.

This is a fast paced position geared toward helping clients engage in the recovery process and become a part of their community. Elder Protective Service Worker

  5. Develop and implement service plan within 5 days of case opening date. Maintain concise and objective progress notes.
  6. Participate in on-call crisis team and 24-hour coverage.
  7. Complete all statistical reports related to position.
  8. Achieve an acceptable level of proficiency in computer programs.

Requirements/Qualifications


QUALIFICATIONS:

  1. Ability to speak Spanish or another foreign language required.
  2. A Masters degree plus at least one year of experience in counseling, casework, or case management preferably in a Protective Services or crisis intervention capacity; OR Bachelors plus at least two years experience.
  3. Excellent communication, interpersonal and organizational skills.

General Overview:

Responsibilities:

Screening and Assessment

  1. Conduct, primarily for medical and surgical inpatients referred to the Consultation Service, SA assessment and diagnosis, including SA and psychiatric treatment history, evidence of active withdrawal syndromes, and current use assessments
  2. Identify patient needs, including stage of change and readiness
  3. Identify high risk bio-psychosocial and patient/family factors that impact treatment and discharge planning

Treatment Planning and Implementation

Clinical Social Worker

Bilingual Family Interventionist/Communication Specialist (Part-time, Contract position)

Background:
The SUPPER project is a brief intervention study funded by the National Institutes of Drug Abuse. The study aims to improve communication skills between parents and their children in order to promote the long-term goal of preventing substance abuse among children as they enter adolescence.

Job Description:
The role of the interventionist will be to facilitate one-on-one informational sessions with parents of pre-adolescent youth (9 to 12 year olds) in Boston. Each session will last approximately one hour. Parents will receive a gender-specific handbook on communication and monitoring that they will be instructed to read before attending the in-person session. The interventionist will discuss the information provided in the handbook and will counsel parents on how to foster communication with their pre-adolescent children, specifically in regards to avoiding drug and alcohol use. The interventionist will also answer questions as-needed based on the information provided in the parent handbook.

Job Description

Perkins is a comprehensive social service agency providing a range of programs and services to meet family and community needs. We are currently seeking therapists for our outpatient group practice. Provide assessment as well as individual, group and family therapy to diverse client base of children, adolescents and adults at area schools and from the community.

Requirements/Qualifications

LCSW, LICSW, LMHC OR LICD, as well as CANS certification. Position begins when site-specific credentialing has been completed.
