Continuing Education Registration Form

NOTES:

Costs

See the workshop list for the cost of each course. Course cost includes a nonrefundable 30 percent registration fee and all course materials. If an institute must be canceled by Simmons College, you will be notified and your money will be fully refunded. Cancellations by registrants must be received in writing — via e-mail or fax 617-521-3192 — five business days prior to the workshop date in order for registrant to receive the maximum refund of 70 percent. Registrants may be subject to a 30 percent penalty fee if course payment is not received in full by the workshop date.

Tax Deduction

A tax deduction is allowed for educational expenses (including registration fees, travel, meals, lodging) if courses are undertaken to maintain and improve professional skills (see Treasury Reg. 1 162-5 Coughlin vs. Commissioner, 203 F.2D 307).

Payment

At this time, we do not accept payments by credit card. For a list of payment methods we do honor, visit our FAQ section.

Participants will not be registered officially until payment is received in full. If payment is not received in full by the date of the workshop, the participant may be subject to a 30 percent penalty fee.

TIP: If your employer is going to pay, or if you are concerned that we will not receive your registration in time, we recommend that you fax your completed registration form with a notation "payment to follow" to our office ASAP. (Fax number: 617-521-3192).
