Cambridge-based Center For Family Connections (CFFC)—a leading provider of support services to adoption, foster care, and complex blended families* in Greater Boston—offers a program for the provision of competent rehabilitative and clinical services to children and adults in complex blended families to produce well-being, safety, and permanency through family support, parent support and youth/adolescent support services. Children who have experienced trauma need a sense of security, safety and permanence. Through its consultations, treatment and group programs, CFFC can make a huge difference in the outcomes for children, adolescents and youth aging out of the system, and their main families. CFFC stresses collaborative planning, concurrent planning, and a systemic perspective, and includes all involved professionals to tap the important knowledge that each of them brings to the life of a child and family.

Fee For Service Clinician (Independent Contractor)

•Reports directly to CEO
•Sees a caseload of clients and complete all appropriate paperwork in a timely fashion
•Works as a team member to meet project objectives
•Perform additional related duties as required
•Attends two meetings per week
•Expected to attend CFFC trainings (CEUs available)
•Receives supervision (individual and group)
•Participates in on-call phone rotation


•Licensed to practice in social work, expressive therapy, psychology, or marriage and family therapy.
•Five or more years of experience, preferably with adoption related work.
•Knowledge of HIPAA standards.
•Substantial experience and knowledge in current clinical principles and practices in adoption consulting and training.
•Demonstrated skills in effective communication and organization.

•Must meet with 8-18 clients/week
•Full Fee For Service plan to be explained at interview.
•Job to start immediately

•Licensed to practice in social work, expressive therapy, psychology, or marriage and family therapy.
•LICSW, LCSW, LMFT, or LMHC (License IS required)
•Five or more years of experience, ideally with some experience in the following: trauma, loss, foster care, kinship care, or adoption.
•Knowledge of HIPAA standards.
•Substantial experience and knowledge in current clinical principles and practices.
•Demonstrated skills in effective communications and organization.
•Clinical paperwork efficiency.

The Care Manager, Social Worker

Job Description

Brattleboro Memorial Hospital has an immediate need for a Care Manager, Social Worker. The Care Manager, Social Worker coordinates and monitors a patient's plan of care; identifies and evaluates a patients' individual needs; and assists patients, families and health care professionals in developing a plan for post-hospital/community care. Provides expert consultation to care providers/treatment team on such
issues as psychiatric illnesses and their treatment. Also serves as an advocate for patients and families and as a resource and educator for hospital and medical staff. Assessment and consultation occurs in all
hospital units, the Emergency Department, medical offices, and outpatient clinics of the hospital.


Requires a graduate degree in Social Work with an active Vermont LICSW, or license eligible. Experience in mental health counseling, including psychosocial and substance abuse assessments, and development of treatment plans. Experience in a hospital setting and experience with briefer treatment modalities. Must demonstrate expertise regarding psychosocial factors affecting patients, substance abuse disorders, family systems, behavior management theory and violence management strategies. Requires critical thinking ability to strategize interventions and treatment options. Must possess excellent organizational skills, ability to work independently, and ability to deal effectively and compassionately with patients and families.
