Clinician- Home Based Services

Job Description

As a clinician employed by Walker working in our Home Based Program you will be providing intensive therapy for a child and family to treat the child's behavioral health needs and help the family to support the child in the home. Families decide on the most appropriate initial service, independently or in consultation with helping professionals such as primary care physicians, mental health clinicians, teachers and school-based staff, case workers, staff members from community organizations, faith leaders, or others.

Hours-This is a full time position. Flexible schedule required. Some evenings and weekends required.

Summary of Position: Provide clinical case management, home based clinical work and other
clinical services to students and families; provide clinical consultation to direct care and educational staff; provide ongoing assessment, treatment planning, and goal focused treatment implementation. This position requires accurate documentation of the client's clinical progress and strong ability to multi-task several cases at once.


Required- M.S.W, Valid drivers license to drive in Massachusetts

Preferred-L.C.S.W, Bi-lingual candidate strongly preferred.

Experience: Required experience in therapeutic treatment with children and their families, including home-based work, proficiency in use of computer systems for clinical information management, excellent oral and written communication skills, and demonstrated ability to produce written reports in a timely manner.

Preferred-Experience with assessment, goal focused treatment planning, TCI (therapeutic crisis intervention) and evidence based treatment modalities. Previous clinical work in special education, family based treatment or residential treatment.

Part-Time Case Manager/Referral Specialist

Hiring Company: Phoenix Charter Academy
Location: Chelsea, MA

Job Description

A part-time Case Manager/Referral Specialist is needed to provide resources and assistance to high-risk teenagers at a charter high school in Chelsea. This position supports the work of the social work department and can be up to 25 hours per week. Clients are adolescents ages 14 to 22 enrolled as full-time students with multiple case management needs, including housing, physical and mental healthcare, public benefits, and/or other community-based services.

Job responsibilities include:

- coordination with Department of Transitional Assistance and MassHealth on behalf of students
- connect students to resources including shelters, Teen Living Programs, mental health counseling, substance abuse treatment, educational support, recreational activities, and healthcare
- assist wtih relevant intakes and reporting, including counseling intakes and Medicaid paperwork


The skills needed for this position include:
- excellent rapport with and dedication to working on behalf of young people;
- be a strong advocate for young people and their families within public systems;
- knowledge of public benefits, including WIC, food stamps, daycare vouchers, cash assistance, MassHealth
- familiarity with Boston area resources, particularly in Chelsea, Everett, and Revere

Ideal candidates for this job would have a degree in Social Work (MSW preferred, but BSW with relevant experience okay). Spanish-speaking candidate strongly preferred and candidates of color are encouraged to apply.
