The Study Therapist delivers group therapy intervention to study participants, and assists with other research responsibilities as part of a pilot randomized controlled trial with HIV-positive Black men who have sex with men (MSM). The study targets medical mistrust and discrimination experiences based on race, sexual orientation, and/or HIV status.

Primary responsibilities include: implementing a skills-based, manualized, cognitive behavioral intervention to reduce medical mistrust and increase functional coping with discrimination experiences among study participants; assisting with recruitment and retention efforts including assessing potential participants, and establishing, revising, and evaluating retention plans; assisting with the publication of research findings through editing, writing, and preparing manuscripts for publication and conference presentations, and presenting at local conferences, meetings and events. Secondary responsibilities include: maintaining study related paperwork and documentation; maintaining participant confidentiality; participating in weekly clinical and/or administrative supervision; maintaining current knowledge of HIV research; and assisting with grant writing and study progress reports.

Clinical Coordinator of Innovative Jail Diversion Program

Anticipated Schedule: generally M-F, can include night and/or weekend shifts

Job Summary:
The Clinical Coordinator of the Jail Diversion Program (CCJD) is primarily responsible for providing out-reach and follow- up for individuals identified for jail diversion.  This program provides services primarily to adults with chronic mental illness whose illness is the primary reason for their involvement in the criminal justice system.  Services may include supportive assessments, development of community support meetings for adults age 18+ with varying levels of impairment, and serving as a liaison to connect individuals with community resources with a framework of recovery.  Position will be based at the Police Department but will require frequent travel in the Somerville area to meet with clients in the community to conduct follow-up and outreach and represent the program at court proceedings.  Position will also staff quarterly Community Crisis Intervention Team meetings and organize as need case reviews for challenging situations.   

Administrative duties include maintaining an active referral data base; conducting research to maintain current best practice on Jail Diversion programming; work with Police Crime Analyst to maintain and improve data collection; gather input from community providers and police officers to assess needs and improve access services and self care supports.  

Essential Job Duties and Responsibilities (may include some or all of the following, based on modality)  

  • Conduct assessments for clients referred, and write necessary documentation.
  • Collaborate with community partners to provide group counseling to at risk clients regarding mental health and substance abuse. Groups can include educational, life skills, relapse prevention groups, relaxation and stress management.
  • Provide crisis intervention when necessary.
  • Coordinate and schedule case intervention reviews and engage in care consultation regarding specific clients with other staff, relatives and appropriate representatives of other agencies, including social service agencies, housing authorities, health care providers, courts and vocational issues.
  • Make appropriate referrals and provide monitoring for compliance.
  • Maintain active database of community resources including treatment and support programming.  Database will be updated on a monthly basis.
  • Coordinate and facilitate quarterly CCIT meetings.  Conduct outreach and community relations to broaden CCIT membership and awareness of program.
  • Take minutes and set quarterly agenda for CCIT meetings.

Requirements/Qualifications

Education and/or Experience Required

  • Bachelors Degree required, Masters Degree preferred; BSW/MSW, CADAC/LADAC, LMHC, or equivalent or actively pursuing certification in counseling, social work or psychology.
  • Bi-lingual in Portuguese, Spanish and or Haitian Creole preferred.
  • Experience in individual and group counseling with individuals with dual-diagnosis.

Skills Required

  • Good written and verbal skills, computer skills, record keeping, and organizational skills.
  • Ability to follow and enforce the policies and procedures of the program.
  • Be able to work independently and be responsible.
  • Familiarity with Excel, Power Point and Microsoft office software and ability to learn QED and CAD police specific database systems.

SENIOR ACTIVITIES COORDINATORTOWN OF WELLESLEY

The Town of Wellesley is seeking a full time Senior Activities Coordinator to plan and oversee recreational and educational activities that the Council on Aging (COA) sponsors for senior citizens. The incumbent also updates and submits a weekly column to the local newspaper and prepares the bi-monthly newsletter. The work requires significant attention to detail, organizational skills, writing skills, and knowledge of departmental practices and procedures. The incumbent often serves as the liaison from the COA to various community groups and works collaboratively with community partners to plan and organize co-sponsored events. Some weekend and evening hours are required for events and day trips. PRINCIPLE DUTIES AND RESPONSIBILITIES

Clinical Components

  • Conduct in-home environmental assessments, Working with families to identify and reduce/remove known or potential triggers from their environment. 
  • Evaluate seif«management skiìls using a standard questionnaire
  • Teach key educational messages in person and over the phone.
  • Develops a Home Action Plan ancL/or disease management plan based on prescribed protocois to 'improve  enviro'mnentaì quality of each home
  • Record and monitor the participants' progress toward goals within speciñe timeframes

Documentation

  • Clearly documents all patient l' family responses on approved protocol forms
  • Documents all activitiesfobservatìonfencounters in the clíenfs record.
  • Provides a Written summary of  after each home visit to the referring plj timely manner. ­Under supervision of Project Manager, communicates findings potentially harmful to the ehild's health and/or safety to the referring provider
  • Enter results of questionnaires into established HIPPA compliant databases

Care Coordination and Case Management

  • Communicates with collateral contacts to improve the quality of health care of patients.
  • Provide equipment and supplies to each client and/0r the client's family to improve disease self-management. Assist with monitoring of supplies
  • Reinforce educational messages regarding disease self-management by linking clients with ive community services and programs. 
  • Offer assistance with referrals to medical care for chronic iilness and other needed services, including tobacco cessation programs when appropriate.
  • Coordinate services initiated and/or involved to meet needs of farnily's treatment plan.

Participate with other staff in community outreach  presentations to community organizations, deveìopment of materials, and phone calls to focused populations

Develops and maintains knowied ge of current trends and practices through participation in required and selected meetings, educational programs, or seminars 5.

Perform other duties as assigned or as necessary 6.

EDUCATION

  • High School dipioma or equivalent required.
  • Bacheìor's Degree preferable in Humans Services related  (nursing, sociai work, health sciences, public health, education, sociology, Asian studies, etc)

EXPERIENCE

Minimum of 2 years prior heaith care relevant experience, preferably in a community setting.
Profîcieney in database management, preferable.
